*This is only an outline of the plan’s features. Hospitalization (admitted as an inpatient) is a requirement to be eligible for some services.All services must be arranged by EA+. Of course, your COA Accident benefits are paid regardless of any other insurance you may have. And activating this safety net is easy. There is NO physical exam. NO health questions. Accidental Death & Dismemberment Reduction on and after Age 70: On the Policy Anniversary Date or next following an Insured Person’s attainment of age 70, his or her amount of Principal Sum will reduce to 50% and upon the attainment of age 75, will reduce to 25%. This reduction will also apply to family coverage.
Family coverage is a percentage of your coverage. The amount of Principal Sum for each of your Covered Dependents is determined below as a percent of your Principal Sum.

Your covered spouse may qualify for up to a $5,000 Education Benefit if enrolled in an Occupational Training program within one year of your death. Certain restrictions described in the Certificate apply.

*All benefits will reduce to 50% when the insured person attains age 70, and an additional 50% at age 75.
ACCIDENTAL DEATH AND DISMEMBERMENT BENEFIT:
If your Injury results in any of the following losses within 365 days* after the date of accident, we will pay the sum shown opposite the Loss in the Loss Table shown below. We will not pay more than the Principal Sum for all losses due to the same accident.

Terms
When Coverage Begins
Your coverage will become effective on the first day of the month following the Administrator’s receipt of your enrollment form and first premium payment.
When Coverage Ends
Coverage for you terminates when the Master Policy is terminated, when you fail to pay the required premium, or when you cease to be an Eligible Person. Coverage for your spouse terminates when he/she is no longer eligible due to marital status, failure to pay premiums, or your coverage ends.
Exclusions
The policy does not cover any loss resulting from:
- intentionally self-inflicted injury, suicide or attempted suicide; whether sane or insane (in Missouri while sane);
- war or act of war, whether declared or undeclared;
- injury sustained while in the armed forces of any country or international authority;
- injury sustained while riding On any aircraft except a Civil or Public Aircraft, or Military Transport Aircraft;
- injury sustained while riding On any aircraft:
- as a pilot, crew member or student pilot;
- as a flight instructor or examiner; or
- if it is owned, operated or leased by or on behalf of the Policyholder, or any employer or organization whose eligible persons are covered under the policy.
Injury means bodily injury resulting directly from accident and independently of all other causes which occurs while you or your spouse / children are covered under the policy.
Loss resulting from:
- Sickness or disease, except a pus-forming infection which occurs through an accidental wound; or
- medical or surgical treatment of a Sickness or disease;
is not considered as resulting from Injury.
Loss means with regard to:
- hands and feet, actual severance through or above the wrist or ankle joints;
- sight, speech or hearing, entire and irrecoverable loss thereof;
- thumb and index finger, actual severance through or above the metacarpophalangeal joints.
This policy provides ACCIDENT insurance only. It does NOT provide basic hospital, basic medical or major medical insurance as defined by the New York State Department of Financial Services.
